At Lockwood, our staff realizes that your child's classroom placement is important to you and that all students have different learning styles and needs. We are providing this parent input form for you to share any specific learning needs, strengths, or conditions you would like considered in the 2023-24 classroom assignment process. When filling out the form, please note the following:

• This information is NOT confidential. This form will be shared with staff during classroom assignment meetings.

• We cannot accept specific teacher requests. Please do not give a specific teacher name.

• We cannot guarantee placement in specific classrooms or with specific peers.

Our goal is to ensure that all Lockwood students have equal opportunity for a positive and strong educational experience. In determining placement, the teacher, grade level team, and administration spend significant time carefully considering your information, our observations at school, and our priority of creating balanced classrooms. Completion of this form is OPTIONAL; all students will be given the same considerations when being placed in a class whether or not parent input is received.
